AdventHealth set out to reduce delays in post-acute transitions and improve care management workflows across its multi-state network of more than 50 hospitals. Leaders knew there were inefficiencies in their PAC referral and discharge processes, but they lacked the real-time data needed to identify bottlenecks, improve accountability, and meaningfully impact ALOS. To solve this, they looked for a centralized platform to streamline workflows, strengthen visibility across teams and PAC partners, and support measurable improvements in patient flow and operational efficiency.
After a five-month ramp-up period, AdventHealth’s new discharge planning tools helped teams care more efficiently – returning valuable time and resources to patients and communities while reducing overall costs by a projected $71 million* annually.
*$71 million is a projected annualized savings based on four months of cost avoidance across the AdventHealth system after an initial five-month ramp up period with Aidin. The savings estimate is based on an Average Length of Stay comparison of July – October 2024 with July – October 2025.
