ProHealth Care, a community health system of 2 hospitals and 300 beds in Waukesha, WI, serves 18,000 discharges annually.
Situation
ProHealth Care had done significant work to improve quality of selected post-acute care facilities in an effort to reduce re-admissions and prepare for bundles, but given regulatory restrictions on patient advising and a lack of comparison data, patients chose care facilities largely based on geography and continued to re-admit despite the Health System’s efforts.
Action
ProHealth Care contracted with Aidin to manage its care transitions. Aidin identified additional high quality post-acute providers with whom ProHealth Care could regularly connect. They streamlined and automated the process for connections and match identification. Aidin continuously collects data on post-acute performance and provides it at the bed-side to drive patient decisions.
