St. Luke’s needed to improve capacity across its network. Leadership knew there were inefficiencies in post-acute care workflows, but they lacked the structured insights and seamless platform to clearly identify where delays were happening or how to fix them.

In 2022, St. Luke’s partnered with Aidin to support discharge planning and Medicare regulatory compliance workflows, including IMM and MOON forms.
Using Aidin, the health system was able to:
● Expedite discharges to post-acute care.
● Gain visibility into bottlenecks in the discharge process.
● Drive accountability internally with staff and externally with post-acute partners.
● Benchmark PACs against other Aidin organizations and identify which PAC partners were underperforming.
● Give patients better information when choosing a PAC provider.
Aidin’s bi-directional Epic integration also removed manual upload steps and made it easier for case managers to share relevant clinical information with post-acute providers at discharge. Management teams gained direct access to status and compliance reports, while frontline users received a clearer view of where each patient was in the transition process.
